Who should I contact regarding reporting unsafe conditions, and injuries or illnesses in relation to your possible workplace lawsuit?

Answer: Report unsafe conditions by calling 1-800-746-1553 or using the online filing links for hazardous conditions. Report accidents, injuries, or illnesses to MSHA using Form 7000-1 online.

Where can I find an accident report from 20+ years ago?
